Hospice Program Director

Napa Valley Hospice & Adult Day Services has an immediate opening for a full-time Hospice Program Director. In this position, you would be responsible for administration, management, and long-range planning of the Home Care and Hospice Departments. You will manage multiple disciplines which may include nursing services, physical therapy, occupational therapy, speech therapy, social work, respite care, and home helpers. You will represent the organization in the service area and direct a coordinated team effort for patient care and customer service. Directs, supervises and governs the home care and hospice programs including in-patient care, home care, and bereavement follow-up. Administers and maintains quality assurance, and environmental policies in accordance with objectives, polices, and procedures. Prepare reports regarding the activities of the operation. Directs and supervises the work of staff in providing for the care and needs of patients. Develops, maintains, and implements policies and procedures. Hires, trains, and motivates a qualified staff. Confers and cooperates with management personnel in formulating administrative and operational policies and procedures. Reviews and analyzes expenditure, financial, and operations reports to determine requirements for assisted living operations. This position reports to the President & CEO.

The successful candidate will be a proven team player with exceptional communication and analytical skills. Excellent computer skills, including Excel, Word, Access, Outlook, and database management are required.

Requirements:

  • Bachelor’s degree (B.S.) from a four-year college or university, including educational program approved for community health nursing by National League for Nursing; or three years related experience and/or training including one year as a Registered Nurse in Hospice.
  • Must possess RN License in California, BSN preferred.
  • Previous supervisory experience preferred or equivalent combination of education and experience.
  • Possession of a valid California State Driver’s License and ability to maintain automobile insurance.

Hospice Registered Nurse (per diem)

Summary: Work with patients to administer and coordinate total nursing care for patients on Hospice service.

Requirements:

  • Current RN license from the State of California.
  • Minimum of one year experience as a Registered Nurse required.
  • Possession of a valid California Drivers License and the ability to maintain automobile insurance.
  • Ability to meet the essential physical demands and functions of the job.
  • Ability to meet requirements of CPR and other mandated programs.
  • Adult Day Services - Physical Therapy Assistant (per diem)

    Requirements:

    • Graduate from an approved school of Physical Therapy Assistant program and be licensed to as a PTA to practice in California.
    • Minimum experience of one year, work experience with a variety of persons with physical therapy or rehabilitation needs.
    • Possession of a valid California Drivers License and the ability to maintain automobile insurance.
    • Ability to meet requirements of CPR, First Aid and other mandated programs.
    • Adult Day Service – Program Aide

      Our Adult Day Services department is seeking per diem Program Aide to assists participants with personal care, toileting, meal service, activities and other duties as directed by the RN Supervisor. Please apply if you possess the following job requirements:

      • Certified Nursing Certificate, a High School Diploma and Training in Personal Care Assistance required.
      • A minimum of one (1) year work experience in providing personal care with elderly persons and disabled adults preferred.

      • Ability to meet requirements of CPR, First Aid and other mandated programs

      Adult Day Service – Activity Coordinator

      Our Adult Day Services department is seeking a full time Activity Coordinator that is responsible for developing individual and group recreation therapy and for planning, organizing, and directing individual and group recreational and social programs that are appropriate for each participant's needs. Also helps to coordinate student interns and community volunteer program. Please apply if you possess the following job requirements:

      • Occupational therapist, art therapist, music therapist, dance therapist, occupational therapist or qualified social worker.
      • A minimum of two (2) years experience in a social, recreational or educational program within the past five (5) years; one (1) year of which was full-time employment in a patient activities program in a healthcare setting, mental health setting or setting for the care of the handicapped.
      • Have completed at least 36 hours of training in a course designated specifically for this position and receive regular consultation for an occupational therapist, occupational therapy assistant, recreation therapist, music therapist, or dance therapist who has had one (1) year of experience in a health care setting.
      • Possession of a valid California Drivers License and the ability to maintain automobile insurance.
      • Ability to meet requirements of CPR, First Aid and other mandated programs
